 Tooth Town In-House Dental Membership Plan – Terms & Conditions  

The Tooth Town In-House Dental Membership Plan is a yearly membership program designed to help families save on dental care. This plan is not dental insurance.

Membership is valid for 12 months from the enrollment date. Benefits do not roll over.

Membership fees are due at enrollment or through approved monthly payment options. Fees are non-refundable once services have been used.

Only services listed as included in the plan are covered at no additional cost. Other services may receive the stated discount.

Some services may not be covered or discounted, including cosmetic treatments, specialist or hospital services, sedation services, and products sold by the office.

Discounts apply only to services provided at Tooth Town Pediatric Dentistry and cannot be combined with other offers.

Plan benefits apply to treatment performed after enrollment.

Included services cannot be exchanged for cash or credit if not used.

Membership is valid only for the enrolled child and is not transferable.

Standard office appointment and cancellation policies apply.

Tooth Town Pediatric Dentistry reserves the right to update or modify the plan benefits, pricing, and terms with notice.
